Bazedoxifene Acetate, Conjugated Estrogens falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here's the gist for Duavee (bazedoxifene plus conjugated estrogens). Both your body and this medicine rely a lot on a process called glucuronidation to package and clear estrogens and bazedoxifene out of your system. A compound found in breadfruit, called artocarpin, has been shown in lab dish studies only to slow that clearing pathway. In theory, that could let hormone levels build up a bit, which might mean more of the usual estrogen-type effects like breast tenderness, bloating, nausea, headaches, or spotting.

The honest part: this has never been reported in real people, and eating breadfruit as a food is very different from taking concentrated leaf teas or extracts. Food amounts are unlikely to cause trouble. Still, before using any breadfruit supplement alongside Duavee, run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Bazedoxifene Acetate, Conjugated Estrogens also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the simple version for Duavee (which combines conjugated estrogens with bazedoxifene). Your liver uses an enzyme called CYP3A4 to help break down and clear the estrogen part of this medicine. Lab and animal studies suggest a compound in breadfruit may gently slow that same enzyme down. If that happened in your body, the estrogen could clear a little slower and build up slightly, which in theory could nudge up estrogen-related effects like breast tenderness, bloating, nausea, or spotting.

The good news: this is rated a minor, theoretical interaction. It's based only on test-tube and animal data, and we don't know if eating or sipping breadfruit actually changes Duavee at all. Breadfruit as a regular food is very unlikely to matter. If you're thinking about concentrated breadfruit leaf teas or extracts, just run it by your pharmacist or doctor first.

What Is Estrogen and Bazedoxifene Used For?

Brand name: Duavee

  • Estrogen and bazedoxifene tablets are used to treat hot flashes (sudden feelings of warmth, especially in the face, neck, and chest) in women who are experiencing menopause (stage of life when menstrual periods become less frequent and stop and women may experience other symptoms and body changes). Estrogen and bazedoxifene tablets are also used to prevent osteoporosis (condition in which the bones become thin and weak and break easily) in women who have undergone menopause. Estrogen is in a class of medications called hormones and bazedoxifene is in a class of medications called estrogen agonist–antagonists. Estrogen works by replacing estrogen that is normally produced by the body. Bazedoxifene is used to block the action of estrogen on the lining of the uterus, decreasing the risk of overgrowth that may lead to cancer.
