Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Caffeine interacts with.

Theoretically, using caffeine with diuretic drugs might increase the risk of hypokalemia.
Caffeine, especially in excessive amounts, can reduce potassium levels due to stimulation of the sodium-potassium pump. Diuretics can also cause lower potassium levels.

Here's the interesting twist with your combo pill: amiloride is actually a potassium-sparing water pill, which is why it's paired with hydrochlorothiazide. The amiloride part is meant to hold onto potassium while the HCTZ tends to flush it out, so the two are designed to balance each other. The concern on file is that large amounts of caffeine could theoretically nudge potassium lower by pushing it into your cells.

In real life, this is a weak, theory-based worry, and your amiloride is built to protect potassium. A normal coffee or tea habit is very unlikely to cause trouble. If you regularly down energy drinks or lots of strong coffee, or you notice muscle weakness, cramps, or an irregular heartbeat, mention it to your pharmacist or doctor so they can check your potassium.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
