CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

Effient falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Cannabidiol (cbd) interacts with.
Research shows that cannabidiol inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabidiol with CYP2C19 substrates may increase the risk for adverse effects from these substrates.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's a twist worth knowing about. Effient (prasugrel) is what we call a prodrug, meaning it's inactive until your body switches it on. While CYP2C19 plays a small part in that activation, prasugrel relies mostly on other enzymes, so it doesn't depend heavily on CYP2C19 the way clopidogrel (Plavix) does. Because of that, CBD blocking CYP2C19 is less likely to meaningfully weaken Effient than it would Plavix.
The interaction on file is real and well-studied for other CYP2C19 drugs, but for Effient specifically the effect is likely minor. Still, since this medicine protects your heart and stents, don't guess. Check with your pharmacist or cardiologist before adding CBD, and be honest about the product and dose you're considering.

In vitro and animal research shows that cannabidiol inhibits CYP2C9. In human studies, cannabidiol has been associated with an increase in plasma levels of topiramate and losartan, CYP2C9 substrates. Concomitant use of cannabidiol with CYP2C9 substrates may increase the risk for adverse effects from these substrates.

Here's something important to know about pairing CBD with Effient (prasugrel). Effient is what we call a prodrug, meaning it's inactive until your body's liver enzymes switch it on. CYP2C9 plays a role in that activation step. Since CBD can slow down (inhibit) CYP2C9, it could theoretically blunt how well Effient gets turned into its clot-fighting form, which would mean less protection against the very clots, heart attacks, or strokes you're trying to prevent.
I'll be honest, the solid human evidence so far is for other CYP2C9 drugs like losartan, not Effient specifically, so this part is more theoretical for your medicine. But because Effient guards against life-threatening clots, I wouldn't take chances. Please loop in your pharmacist or cardiologist before adding any CBD product.

In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's something important about Effient (prasugrel): it's actually a prodrug, meaning you swallow it in an inactive form and your body has to switch it on. While CYP3A4 plays a role, prasugrel relies heavily on several enzymes (including esterases and CYP3A4) to become active, and once it's switched on, it stays active because it binds platelets permanently. So blocking CYP3A4 a little is not likely to make Effient dangerously strong the way it would for a drug that just builds up in your blood.
That said, anything that nudges how a blood thinner works is worth a conversation. CBD products also vary a lot in quality and dose. Before adding CBD, please check with your pharmacist or doctor so they can weigh your bleeding risk and watch for easy bruising or unusual bleeding.

In vitro research shows that cannabidiol inhibits CYP2B6. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2B6 substrates might increase the risk for adverse effects from these substrates.

Here's the thing to keep in mind: Effient (prasugrel) is a prodrug, meaning your body has to activate it before it can thin your blood and protect against clots. The flag on file is that CBD may block an enzyme called CYP2B6, which is one of the players involved in handling this medication. So in theory, if that pathway slows down, you could end up with more active drug around than expected.
With a blood thinner like this, more activity could mean a higher chance of bruising, nosebleeds, or harder-to-stop bleeding. I want to be honest, though: this is only a theoretical concern based on lab studies, and it hasn't actually been reported in real patients. Still, since bleeding risk is serious business, please loop in your pharmacist or doctor before pairing CBD with Effient.

What Is Prasugrel Used For?
Brand name: Prasugrel
Prasugrel is used to reduce blood clot–related heart events, including clots forming in a stent (stent thrombosis). It is for people with acute coronary syndrome who are going to be treated with percutaneous coronary intervention (PCI, a procedure to open blocked heart arteries).
This includes people with unstable angina or a non-ST-elevation heart attack (NSTEMI), and people with an ST-elevation heart attack (STEMI) treated with primary or delayed PCI. Both Effient and generic Prasugrel tablets carry this use.
