Here's the thing to know if you use cannabis and take omeprazole: the THC in cannabis gets cleared out of your body partly by a liver enzyme called CYP2C9. Omeprazole can mildly slow that same enzyme down. In theory, that means THC could hang around a little longer than usual, so you might feel its effects (drowsiness, dizziness, a stronger high, dry mouth, or feeling foggy) more than you expect.

I want to be upfront, though. This is a theoretical concern based on how the enzyme works, not something proven in real-world studies. Omeprazole is only a weak inhibitor here, so for most people the effect is likely small. Still, if you notice cannabis hitting harder than usual, mention it to your pharmacist or doctor before changing anything.

Theoretically, cannabis might increase the levels and adverse effects of CYP2C9 substrates.
In vitro research shows that the cannabis constituents delta-9-tetrahydrocannabinol (THC), cannabidiol (CBD), and cannabinol moderately inhibit the CYP2C9-mediated 7-hydroxylation of S-warfarin in a concentration-dependent manner. In vitro research also shows that cannabis extracts modestly inhibit the CYP2C9 metabolism of tolbutamide; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist: your body uses a liver enzyme called CYP2C9 (along with another one, CYP2C19) to help clear omeprazole out of your system. Lab studies suggest that compounds in cannabis, including THC and CBD, can partly block CYP2C9. In theory, that could slow how fast omeprazole is broken down, letting a little more linger in your blood.

Honestly though, this is mostly a theoretical concern from test-tube research, not real patients. Omeprazole is also a pretty forgiving medication, so any small bump usually wouldn't cause big problems, maybe slightly more of the typical effects like headache, nausea, or stomach upset. I wouldn't lose sleep over it, but if you use cannabis regularly and take omeprazole daily, mention it to your pharmacist or doctor so they can keep an eye on things.

In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the idea behind this one. Your body uses an enzyme called CYP3A4 to help clear omeprazole from your system. In lab dish studies, compounds in cannabis (especially CBD and CBG) appear to slow that enzyme down a bit. If that happened in real life, omeprazole could hang around a little longer and its levels could creep up. Omeprazole is generally well tolerated, but more of it could mean things like headache, stomach upset, gas, or diarrhea.

I want to be honest about the strength here: this is only test-tube and animal evidence, not proven in people, so for most folks it's likely minor. Omeprazole isn't a finicky narrow-window drug, so I wouldn't lose sleep over it. Still, mention your cannabis use to your pharmacist or doctor so they can keep an eye on things.

Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
