Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to clear brexpiprazole (Rexulti). Lab studies hint that cascara sagrada might gently rev up that enzyme, which could mean your body breaks down the Rexulti a bit faster. In theory, that might leave slightly less of the medication working, so symptoms it controls (like mood or thinking) could creep back.

I'd keep this in perspective, though. This is based only on test-tube research, the effect was far weaker than known enzyme boosters, and it's rated a minor concern. The bigger real-world issue with cascara is that, as a stimulant laxative, it can cause cramping, dehydration, and electrolyte shifts that aren't great alongside any antipsychotic. Before using cascara, check with your pharmacist or doctor.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
