Buproprion, Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Auvelity combines bupropion and dextromethorphan, and one piece of why it works is that the bupropion actually slows down the breakdown of the dextromethorphan, keeping its levels up in your body. The concern on file is that cascara sagrada might gently speed up a liver enzyme called CYP3A4, which could nudge these drugs to clear a little faster and slightly soften their effect on mood.

Honestly, this is a very mild, mostly theoretical worry. It comes only from lab studies, not from people, and the effect was far weaker than known strong inducers. The bigger real-world issue with cascara is that it's a harsh stimulant laxative that can cause cramping, dehydration, and electrolyte shifts. Before pairing the two, just run it by your pharmacist or doctor.
