Fostamatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the situation with these two. Fostamatinib (Tavalisse) is actually a prodrug, meaning your body has to convert it into its active form, called R406, before it can do its job of protecting your platelets. The lab research we have suggests cascara sagrada might gently speed up the CYP3A4 enzyme that helps process this drug. In theory, that could lower the amount of active medicine in your system, which might make it a little less effective at keeping your platelet count up.

That said, I don't want to overstate this. The evidence here is only test-tube data, and the effect was much weaker than known strong inducers. So this is more of a theoretical heads-up than a real-world alarm. Still, since fostamatinib treats a serious platelet condition, it's worth mentioning to your doctor or pharmacist before adding cascara.
