Why this applies to Mometasone

Mometasone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the good news on this pairing. The concern on file is that cascara sagrada might gently speed up a liver enzyme called CYP3A4, which would tend to clear certain drugs a little faster and weaken them. But this is mostly relevant for medicines you swallow that travel through your liver. Mometasone (Elocon) is a cream or ointment you put on your skin, so very little of it reaches your bloodstream or your liver in the first place.

On top of that, this interaction is theoretical, based only on lab studies, and the effect was much weaker than a true enzyme booster like rifampin. For topical mometasone, I wouldn't expect any meaningful change in how well it calms your redness, itching, or eczema. If you're using the inhaled form (Asmanex), just mention it to your pharmacist to be safe.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

Mometasone Furoate topical products are used to relieve the inflammation and itching (pruritus) that come with corticosteroid-responsive dermatoses — a broad category of skin conditions that respond to steroid treatment.

Mometasone Furoate Topical Solution (Lotion) is specifically indicated for these skin conditions in patients 12 years of age and older. Mometasone Furoate cream may be used in adults and in children 2 years of age and older.
