nefazodone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for nefazodone (Serzone). Your liver clears this antidepressant using an enzyme called CYP3A4. Lab studies hint that cascara sagrada may gently speed up that same enzyme. If that happened in real life, your body could break nefazodone down a little faster, which in theory might make it work slightly less well. That could show up as your depression symptoms not being controlled as well as usual.

I want to be honest about how thin this evidence is. It comes only from test-tube research, the effect was much weaker than known strong inducers, and it's rated a minor, possible interaction. Occasional short-term laxative use likely won't upset your treatment. Still, check with your pharmacist or doctor before pairing them, and never adjust your nefazodone on your own.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Nefazodone Used For?

Brand name: Serzone

Nefazodone Hydrochloride tablets are approved to treat depression, specifically major depressive disorder (MDD). Clinical trials established its effectiveness over 6 to 8 weeks in outpatients and 6 weeks in hospitalized patients with depression.

Nefazodone has also been shown to help prevent depression from coming back in patients who responded well to initial treatment. Because of a serious risk of liver failure, prescribers are advised to consider other antidepressants before turning to nefazodone.
