Nimodipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist: your body clears nimodipine using a liver enzyme called CYP3A4. Lab research hints that cascara sagrada may gently speed up that enzyme, which could in theory clear nimodipine a little faster and slightly weaken its effect. Since nimodipine is given to protect the brain after a bleed (a subarachnoid hemorrhage), you really want it working at full strength.

That said, this concern comes only from test-tube studies, and the effect was far weaker than a true enzyme booster like rifampin, so the real-world impact is likely small. Cascara is also a strong laxative that can cause cramping and fluid loss. If you're taking nimodipine, please check with your pharmacist or doctor before using it.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Nimodipine Used For?

Nimodipine — available as capsules and oral solution — is used to improve neurological outcomes after a subarachnoid hemorrhage (SAH), a type of bleeding that occurs around the brain when an intracranial berry aneurysm ruptures. It works to reduce the incidence and severity of ischemic deficits (areas of the brain damaged by reduced blood flow) that can follow this kind of bleed.

This use applies across nimodipine capsules, nimodipine oral solution, and Nymalize oral solution. It is indicated for adult patients regardless of how severe their neurological condition is at the time treatment begins.
