Simeprevir Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for Olysio (simeprevir): your liver uses an enzyme called CYP3A4 to process this medication. Lab studies hint that cascara sagrada might gently nudge that enzyme to work a little faster. If that happened in real life, your body could clear simeprevir a bit quicker, which in theory might leave less of it around to fight the hepatitis C virus.

I want to be honest about how weak this signal is. It comes only from test-tube research, the effect was far smaller than known strong inducers, and we don't have human cases showing it actually matters. Still, because keeping enough simeprevir in your system is the whole point of treatment, I'd run any cascara use past your pharmacist or doctor first rather than guessing.

What Is Simeprevir Used For?

Brand name: Olysio

  • Simeprevir is used along with ribavirin (Copegus, Rebetol) and peginterferon alfa (Pegasys) to treat chronic hepatitis C (an ongoing viral infection that damages the liver). Simeprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Simeprevir may not prevent the spread of hepatitis C to other people.
