Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist with Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) and cascara sagrada. Velpatasvir and voxilaprevir are partly handled by a liver enzyme called CYP3A4. Cascara sagrada, in lab dish studies only, seems to mildly speed up that enzyme. If that happened in your body, it could in theory lower the levels of this medicine, and with hepatitis C drugs you really want full strength so the virus is fully cleared. Weaker drug levels could mean a treatment that doesn't work as well.

That said, this is a minor, theoretical concern based on test-tube work, not real patients, and the effect was much weaker than known enzyme boosters. Still, since finishing your full course matters, check with your pharmacist or doctor before using cascara.

Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
