Why this applies to Ziprasidone

Ziprasidone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the basic idea with these two together. Your liver uses an enzyme called CYP3A4 to help clear ziprasidone (Geodon) out of your body. Lab studies hint that cascara sagrada may slightly speed that enzyme up. If it did, your body could break down ziprasidone a bit faster, which in theory might leave less of it working. For a medication that keeps mood and thinking steady, weaker effects could mean symptoms aren't as well controlled.

That said, I want to keep this in perspective. This is theoretical, based only on test-tube research, and the effect was far weaker than known enzyme boosters. Cascara is also meant only for very short-term use. Still, if you take Geodon, check with your pharmacist or doctor before using any cascara product.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Ziprasidone Used For?

Brand name: Geodon

Ziprasidone is prescribed for adults with schizophrenia and for adults experiencing manic or mixed episodes of bipolar I disorder. It can be used on its own for acute manic or mixed episodes, or added to lithium or valproate to help maintain stability in bipolar I disorder long-term.

An injectable form is also available for rapid control of agitation in people with schizophrenia who need quick relief. Prescribers are advised to weigh ziprasidone's potential effect on heart rhythm when choosing among treatment options.
