Drug & supplement interaction

Cinchona + Venlafaxine

Venlafaxine brand names: Effexor XR, Effexor.

Moderate Interaction
The bottom line Moderate

Venlafaxine falls into 2 drug categories that Cinchona can interact with; the most serious is rated moderate. Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.

Cinchona and Venlafaxine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Cinchona interacts with whole categories of medication. Venlafaxine falls into 2 of those categories, so it can interact with Cinchona in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 2

Why this applies to Venlafaxine

Venlafaxine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cinchona interacts with.

Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body relies on a liver enzyme called CYP2D6 to help break down venlafaxine (Effexor). Cinchona bark contains quinidine, which is known to block that exact enzyme. So in theory, pairing the two could let venlafaxine build up higher than usual in your system.

If that happens, you might notice more of venlafaxine's typical effects, things like nausea, jitteriness, trouble sleeping, a faster heartbeat, higher blood pressure, or sweating. To be fair, this concern is theoretical and based on how these compounds behave, not on real-world reports with this combo. Still, cinchona bark is unpredictable on its own, so please check with your pharmacist or doctor before mixing them rather than guessing.

QT INTERVAL-PROLONGING DRUGS

2 of 2

Venlafaxine also falls under QT INTERVAL-PROLONGING DRUGS.

Theoretically, taking cinchona with other QT interval-prolonging drugs might cause an additive effect and increase the risk of ventricular arrhythmias.
Quinidine and quinine, which are constituents of cinchona, prolong the QT interval.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with cinchona and your venlafaxine (Effexor). Cinchona bark naturally contains quinine and quinidine, and both of those can stretch out something called the QT interval, which is basically the timing of your heartbeat's electrical reset. Venlafaxine can also nudge the QT interval a little, especially at higher doses. Put them together and, in theory, those small effects could add up and slightly raise the risk of an irregular heart rhythm.

I want to be honest about how strong this is: it's mostly based on pharmacology, not real-world reports, so it's a theoretical concern rather than something that happens to most people. Still, since cinchona bark itself is unpredictable, please check with your pharmacist or doctor before combining them rather than guessing.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Cinchona

What is Cinchona?

Cinchona
  • Malaria (historical/traditional)
  • Fever
  • Leg cramps and restless legs
  • Digestive upset and loss of appetite
  • Bitter tonic

Cinchona bark is the natural source of quinine and has been used for centuries to treat malaria and fever, but the bark itself is not standardized and can be unpredictable and risky. Modern medicine uses purified, dose-controlled quinine instead, and the supplement form carries real dangers, including serious heart and blood problems. Talk to a pharmacist or doctor before using it, and avoid it during pregnancy.
