Corydalis Yanhusuo and Aspirin, Caffeine, Codeine Phosphate

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Corydalis Yanhusuo interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 4 of those categories, so it can interact with Corydalis Yanhusuo in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 4

Why this applies to Aspirin, Caffeine, Codeine Phosphate

Aspirin, Caffeine, Codeine Phosphate falls under CNS DEPRESSANTS — the group Corydalis Yanhusuo interacts with.

Theoretically, Corydalis yanhusuo might increase the sedative effects of CNS depressants.
Corydalis yanhusuo contains berberine. Animal research suggests that berberine may have sedative effects. Theoretically, Corydalis yanhusuo might also have CNS depressants effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

The piece of this combination product that matters most here is the codeine. Codeine is an opioid that calms the nervous system, and it can make you feel drowsy, foggy, or lightheaded on its own. Corydalis yanhusuo may have its own mild sedating effects, so taking the two together could, in theory, add up and leave you more sleepy or slowed down than usual.

I want to be honest about how strong this concern is. It's based on lab and animal research, not solid human studies, so it's a theoretical caution rather than a proven problem. Still, if you notice extra grogginess, slower breathing, or trouble staying alert, that's worth paying attention to. Please check with your pharmacist or doctor before pairing them, especially since alcohol or other calming medicines would add to the effect.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 4

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, Corydalis yanhusuo might increase serum levels of drugs metabolized by CYP2D6.
Corydalis yanhusuo contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, Corydalis yanhusuo might also inhibit CYP2D6.
Severity Moderate Likelihood Possible Evidence D

Dr. Brian Staiger, PharmD, BCPS

Here's the wrinkle with this combo pill: the codeine part is a prodrug. Your body has to use an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Corydalis yanhusuo may slow that same enzyme down. So in theory, less codeine gets converted, and you could get less pain relief than expected. The aspirin and caffeine pieces aren't really affected here.

I want to be honest about the strength of this: it's theoretical, based on how the herb behaves in lab studies, not solid human data. So I wouldn't panic. Still, if your pain medicine seems to be working less well, that's worth mentioning. Please check with your pharmacist or doctor before pairing these.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 4

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Corydalis yanhusuo might increase serum levels of drugs metabolized by CYP3A4.
Corydalis yanhusuo contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, Corydalis yanhusuo might also inhibit CYP3A4.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about this combo pill: the part that matters most with corydalis is the codeine. Corydalis may mildly slow down a liver enzyme called CYP3A4. Codeine is partly handled by that pathway, so in theory its breakdown could slow a little, leaving slightly more in your system. That might mean more of codeine's usual effects, like drowsiness, dizziness, constipation, or feeling foggy. The aspirin and caffeine aren't really the concern here.

I want to be honest though: this is a theoretical interaction based on lab research, not solid human studies, so it may not amount to much in real life. Still, since codeine affects breathing and alertness, it's worth a quick chat with your pharmacist or doctor before pairing the two, especially if you take other sedating medicines.

ANTICOAGULANT/ANTIPLATELET DRUGS

4 of 4

Aspirin, Caffeine, Codeine Phosphate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, Corydalis yanhusuo might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Corydalis yanhusuo contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, Corydalis yanhusuo might also inhibit platelet aggregation.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind with this combo product: the aspirin part already thins your blood a little by keeping platelets from clumping together. Corydalis yanhusuo contains a compound called berberine, which in lab and animal studies seems to do something similar. Put them together and, at least in theory, you could nudge up the risk of bleeding or bruising.

I want to be honest about how solid this is though. The evidence here comes only from test tube and animal research, not real-world human studies, so it's more of a caution flag than a proven problem. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or stomach upset, since aspirin can already irritate the stomach. Before adding corydalis, it's worth a quick chat with your pharmacist or doctor.
