Here's the thing to keep an eye on if you pair Corydalis yanhusuo with your metformin. This herb may contain berberine, a compound that some studies show can nudge blood sugar lower on its own. Since metformin already works to bring your glucose down, stacking the two could push your levels lower than intended.

I'll be honest, this concern is mostly theoretical, based on how the compounds behave rather than solid human studies in people taking both together. Still, watch for signs of low blood sugar like shakiness, sweating, dizziness, or feeling unusually tired. Before adding this herb, please check in with your pharmacist or doctor, and don't change your metformin dose on your own.

Theoretically, Corydalis yanhusuo might increase the therapeutic and adverse effects of metformin.
Corydalis yanhusuo contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if Corydalis yanhusuo might have this same effect.

Here's the gist for your metformin (Riomet ER). Corydalis yanhusuo may contain berberine, and in lab and animal studies berberine seemed to slow how quickly the body clears metformin. The thinking is that this could let a bit more metformin hang around, which might gently boost both its blood-sugar-lowering effect and its side effects, like nausea, an upset stomach, diarrhea, or that metallic taste. In the studies, this showed up most when berberine was taken about 2 hours before metformin, not when taken together.

I want to be honest with you: this is theoretical, based on pharmacology and animal data, and we don't even know if corydalis acts the same way. It's not a reason to panic. Just give your pharmacist or doctor a heads-up before combining them so they can keep an eye on your numbers.
