Esomeprazole Strontium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the gist for esomeprazole strontium, the acid-reducer you take for heartburn and reflux. Some compounds in cubeb berries have slowed down an enzyme called CYP3A4 in lab dishes. Esomeprazole is handled in the body partly by that pathway (though it leans more heavily on a different enzyme, CYP2C19). In theory, if cubebs slowed that breakdown, a bit more esomeprazole could linger, which might mean slightly stronger effects or side effects like headache, nausea, or stomach upset.

I want to be honest with you: this is only seen in test-tube studies, never in real people, so the practical risk looks small, especially with food-amounts of the spice. Concentrated cubeb oils or extracts are a different story. Before using those, check with your pharmacist or doctor.

Esomeprazole Strontium also falls under PROTON PUMP INHIBITORS (PPIs).

Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of PPIs. PPIs include omeprazole (Prilosec), lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium).

Esomeprazole strontium is a proton pump inhibitor, which means its whole job is to turn down acid production in your stomach. The concern here is the opposite direction of work: some reports suggest cubebs may nudge stomach acid up. If that happens, it could theoretically push back against what your PPI is trying to do, so you might notice your usual heartburn, acid reflux, or sour stomach creeping back.

I want to be honest, though. This is based only on weak, anecdotal information, not solid human studies, and it is rated a minor interaction. A pinch of cubebs as a spice is very unlikely to matter. Concentrated cubeb extracts or oils are a bigger question mark, so check with your pharmacist or doctor before using those regularly.
