Selpercatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Dhea interacts with.

Theoretically, DHEA might increase the levels of drugs metabolized by CYP3A4.
Some preliminary evidence shows that DHEA may inhibit CYP3A4; however, the clinical significance of this potential interaction is not known.

Here's the basic idea: your body clears selpercatinib (Retevmo) largely through a liver enzyme called CYP3A4. Some early lab signals hint that DHEA might slow that enzyme down a bit. If that happened, your cancer medicine could clear out more slowly and build up to higher levels than expected, which could mean more of its known side effects, things like high blood pressure, swelling, diarrhea, fatigue, liver enzyme changes, or effects on heart rhythm (QT prolongation).

I want to be honest about how soft this evidence is. It's theoretical and based on weak, anecdotal data, so we really don't know if it matters in real life. Still, with a serious cancer drug like this, it's worth a quick chat with your oncologist or pharmacist before adding DHEA.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
