Why this applies to Crizotinib

Crizotinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Here's the basic idea with eleuthero and your Xalkori (crizotinib). Your body uses a tiny pump called P-glycoprotein to move drugs like crizotinib out of cells. In lab dish studies, eleuthero seems to slow that pump down. If that held true in people, less crizotinib would get pushed out, so a bit more could build up in your system. That could mean stronger side effects, things like nausea, vision changes, swelling, fatigue, or changes in heart rhythm or liver labs.

The honest part: this comes only from test-tube research, not real patients, so we truly don't know if it matters in everyday life. With a serious cancer medicine, though, I'd play it safe. Please check with your oncologist or pharmacist before adding eleuthero.

Crizotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the short version: your body clears crizotinib (Xalkori) using a liver enzyme called CYP3A4. In test tubes and animal studies, eleuthero looked like it might slow that enzyme down, which in theory could let crizotinib build up a bit and lead to more of its usual side effects, things like nausea, vision changes, swelling, fatigue, or liver irritation.

The good news is that when researchers actually tested eleuthero in healthy people, it did not meaningfully change CYP3A4 activity. So this is rated minor and unlikely, more of a theoretical concern than a proven one. Still, crizotinib is a serious cancer drug, so I'd run any new supplement past your oncologist or pharmacist before starting it.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.

Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
