CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Crizotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Moringa interacts with.
Some in vitro research suggests that moringa inhibits cytochrome P450 3A4 (CYP3A4). However, other in vitro research suggests that moringa extract induces CYP3A4 enzymes. A pharmacokinetic study in patients with HIV shows no change in the pharmacokinetics of nevirapine, which is partially metabolized by CYP3A4, when administered concomitantly with moringa leaf powder 1.85 grams daily for 14 days.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with moringa and crizotinib (Xalkori). Your body relies on a liver enzyme called CYP3A4 to break this cancer drug down, and lab studies suggest moringa could nudge that enzyme in either direction. If moringa slows the enzyme, crizotinib may build up, which could mean more nausea, vomiting, diarrhea, swelling, vision changes, or strain on your heart rhythm and liver. If moringa instead speeds the enzyme up, your crizotinib levels could drop and the drug might not work as well against your cancer.
Honestly, this is mostly theory from test-tube research, and one human study found no real change with a different drug. Still, crizotinib is too important to gamble with, so please run any moringa supplement past your oncologist or pharmacist first. Small amounts in food are far less likely to matter than concentrated capsules.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that moringa leaf extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.

Here is the gist for Xalkori (crizotinib). Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs around and help clear them. Lab studies suggest moringa leaf extract may slow that pump down. If that happened in people, more crizotinib could stay in your system, which in theory could mean stronger effects and more side effects like nausea, vision changes, swelling, fatigue, or liver and heart rhythm concerns.
I want to be honest with you though: this is based only on test-tube research, and it has not actually been seen in humans. Eating moringa as food is unlikely to be a problem, but concentrated supplements are a different story. Since crizotinib is a serious cancer medicine, please run any moringa supplement past your oncologist or pharmacist first.

What Is Crizotinib Used For?
Brand name: Xalkori
Xalkori (crizotinib) treats adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for ALK or ROS1 gene changes, as confirmed by an FDA-approved test.
Xalkori is also used for pediatric patients (age 1 and older) and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL), and for adult and pediatric patients (age 1 and older) with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
