Why this applies to Perphenazine

Perphenazine falls under PHENOTHIAZINES — the group Evening Primrose interacts with.

Theoretically, taking evening primrose oil with phenothiazines might increase the risk of convulsions.
Evening primrose oil contains gamma-linolenic acid (GLA). There is some concern that taking supplements containing GLA might cause seizures, or lower the seizure threshold, when taken with phenothiazines. In one report, three patients with schizophrenia who had received phenothiazines developed EEG changes suggestive of temporal lobe epilepsy after starting treatment with GLA, although none experienced an actual seizure. In another report, two patients with schizophrenia who were stabilized on phenothiazines developed seizures when evening primrose oil 4 grams daily was added. One of these patients had a prior history of seizures. It is unclear whether evening primrose oil had any additive epileptogenic effects with the phenothiazines; there is no evidence that taking evening primrose oil alone causes seizures.

Here's the gist: perphenazine is an antipsychotic that, like other phenothiazines, can slightly lower your seizure threshold on its own. The concern with evening primrose oil is that its main ingredient, GLA, may nudge that threshold down a little further, theoretically making a seizure more likely. In a couple of older case reports, people stable on phenothiazines developed seizures after adding about 4 grams of evening primrose oil daily.

I want to be honest about how soft this evidence is. It's anecdotal, and one of those patients already had a seizure history. For most people this is a low risk, but if you have epilepsy or past seizures, it matters more. Please check with your pharmacist or doctor before combining the two rather than stopping anything on your own.

Evening primrose oil is a seed oil rich in gamma-linolenic acid (GLA), an omega-6 fatty acid, that is popularly used for skin conditions, PMS, and breast pain. The evidence behind most of these uses is mixed or weak, and it is not proven to be a reliable treatment for any condition. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before starting it, especially if you are pregnant or have a bleeding disorder.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
