Fluconazole falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the gist for this pairing. Fluconazole blocks a liver enzyme called CYP3A4, which is one of the pathways that helps clear evodia's active compounds (like evodiamine and rutaecarpine) from your body. So the concern runs in one direction: the fluconazole could slow the breakdown of evodia, letting more of it build up. That might mean stronger effects from the supplement, and potentially more of whatever side effects evodia can cause.

I want to be honest about how solid this is. This interaction comes from animal and test-tube studies only, using a different antifungal, so it's theoretical for people taking fluconazole. It's not a reason to panic, but since fluconazole itself can be hard on the liver, please run it by your pharmacist or doctor before combining them.

What Is Fluconazole Used For?

Brand names: Diflucan, Diflucan Injection

Fluconazole treats several fungal infections. These include Candida infections of the mouth, throat and esophagus (the tube from the throat to the stomach), and cryptococcal meningitis (a fungal infection of the lining around the brain).

Vaginal yeast infections are a labeled use of the tablets (Diflucan and fluconazole tablets) and of products labeled for both tablets and suspension. They are not listed for fluconazole oral suspension on its own or for the intravenous (IV) products. All products are also used to lower the chance of candidiasis in people having chemotherapy and/or radiation before a bone marrow transplant.
