Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Evodia interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and clinical effects of evodia.
Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here the direction is the reverse of what people usually worry about. In this case, Lonafarnib is acting as the CYP3A4 inhibitor, meaning it can slow the liver enzyme that helps break down Evodia's active compounds (like evodiamine and rutaecarpine). So it isn't that Evodia changes your Lonafarnib levels here. It's that your Lonafarnib could let Evodia build up higher and linger longer in your body.

Keep in mind this is only based on test tube and animal work, not human studies, so it's theoretical. Still, since we know little about Evodia in people, I'd play it safe. Please check with your pharmacist or doctor before pairing them, rather than starting or stopping anything on your own.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the gist for Lonafarnib (Zokinvy). Your body relies heavily on a liver enzyme called CYP3A4 to break this medicine down and keep its levels in a safe range. Lab studies suggest evodia may block, or slow, that same enzyme. If that happened in people, lonafarnib could build up higher than expected, which may mean more of its typical side effects like nausea, vomiting, diarrhea, poor appetite, fatigue, and changes seen on heart tracings (QT prolongation).

I want to be honest about the strength of this: it's theoretical, based only on test-tube work, and has not been shown in humans. Still, lonafarnib is a sensitive drug with careful dosing, so don't start evodia without first checking with your doctor or pharmacist.

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
