Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the gist for Zokinvy (lonafarnib). Your body partly relies on a liver enzyme called CYP2C9 to help clear this medicine. Milk thistle might slow that enzyme down a little, and if it did, lonafarnib could build up and bring on more of its usual side effects, like nausea, vomiting, diarrhea, poor appetite, and feeling tired or run down.

The good news is the evidence here is genuinely shaky. Lab studies hint at a slowdown, but solid human studies suggest milk thistle doesn't meaningfully block CYP2C9. So a real-world problem is considered unlikely. Still, since lonafarnib is a specialized drug, please loop in your pharmacist or doctor before adding milk thistle rather than starting or stopping anything on your own.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the gist with Zokinvy (lonafarnib): your body relies heavily on an enzyme called CYP3A4 to clear it out. The worry on paper is that milk thistle might slow that enzyme down a bit, which in theory could let lonafarnib build up and bring on more of its usual side effects, things like nausea, vomiting, diarrhea, loss of appetite, or fatigue.

The good news is the evidence here is thin and conflicting. In careful human studies, milk thistle didn't meaningfully change how similar CYP3A4 drugs were processed, so a real problem is considered unlikely. Still, because lonafarnib is a sensitive drug with strict dosing rules, I'd run any milk thistle plan by the prescribing team or your pharmacist before adding it.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
