Why this applies to Lonafarnib

Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: in a lab dish, red clover looked like it might slow down CYP2C9, one of the liver enzymes that helps clear Lonafarnib (Zokinvy). If that actually happened in your body, the worry would be a little extra Lonafarnib building up, which could mean more of its usual side effects like nausea, vomiting, diarrhea, loss of appetite, or fatigue.

The reassuring part is that this is only a theoretical concern. When researchers actually tested red clover isoflavones in people, they saw no real change in how a CYP2C9 drug was handled. So a meaningful interaction here is unlikely. Still, Lonafarnib is a specialized medicine, so check with your pharmacist or doctor before adding red clover.

Here's the gist for Zokinvy (lonafarnib), which your body clears using an enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, and if it did, lonafarnib could build up. Since lonafarnib is an active drug (not a prodrug that needs switching on), higher levels would mean a greater chance of its usual side effects, like nausea, vomiting, diarrhea, poor appetite, and tiredness.

The reassuring part: this is mostly theoretical. A small human study using a standardized red clover dose showed no real effect on a CYP3A4 test drug. So a meaningful interaction is unlikely. Still, lonafarnib is a specialty medicine with careful dosing, so please check with your pharmacist or doctor before adding red clover.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
