Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the tricky part with this combo product (Hycomine Compound). The hydrocodone piece is a bit like codeine, it relies on a liver enzyme called CYP2D6 to be turned into its stronger, more active form. Lab studies hint that fever bark, through a yohimbine-like compound, may slow that same enzyme down. If that happened, you might actually get less pain relief from the hydrocodone, since less of it gets activated.

I want to be honest though: this is only test-tube evidence, and the real effect of fever bark in people is unknown. On top of that, fever bark's own alkaloids can push around your blood pressure and heart rate, which doesn't mix well with the caffeine and phenylephrine already in this product. I'd skip the fever bark and check with your pharmacist or doctor first.

Here's the twist with this combo: it's not your cold-and-cough medicine that piles up, it's the fever bark that may stick around longer. One of the ingredients in this combination product, chlorpheniramine, can gently block a liver enzyme called CYP2D6. That same enzyme helps clear yohimbine, a punchy alkaloid found in fever bark. With CYP2D6 slowed down, yohimbine levels could theoretically climb.

If that happens, you might notice the kind of effects yohimbine is known for: a racing heart, jitteriness, anxiety, sweating, or swings in blood pressure. Honestly, though, this is a theoretical concern based on how the drugs behave in the lab, not proven in real patients. Fever bark is already a risky herb, so I'd skip it and check with your pharmacist before combining the two.

This combo product (Hycomine Compound) packs in a couple of ingredients that act on your blood pressure and nervous system, namely caffeine and phenylephrine. Phenylephrine is a decongestant that narrows blood vessels and can nudge blood pressure up, and caffeine is a stimulant. Fever bark contains plant alkaloids, including a yohimbine-like compound that can also raise blood pressure, and a reserpine-like compound that can push it the other way. So in theory, pairing them could either add up or work against each other in unpredictable ways.

Honestly, this is a theoretical concern based on weak, anecdotal reports, not solid human studies. Still, fever bark is potent and best avoided. If you take blood pressure meds or have heart concerns, run it by your pharmacist or doctor before combining.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
