Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the wrinkle with Fiorinal with Codeine: the codeine part is a prodrug. Your body needs an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Fever bark contains a yohimbine-like compound that, at least in lab tests, can block CYP2D6. If that enzyme is slowed down, less codeine gets converted, so you might get weaker pain relief than expected rather than more.

Honestly, this is theoretical and based only on test-tube data, so the real-world effect of fever bark is unclear. The other ingredients (aspirin, butalbital, caffeine) aren't the concern here. Fever bark also affects blood pressure and heart rate on its own, so I'd run this combo by your pharmacist or doctor before mixing them.

Here the piece that matters is the caffeine in Fiorinal with Codeine, since caffeine is a mild stimulant. Fever bark carries some unusual plant alkaloids (reserpine-like and yohimbine-like compounds) that can nudge blood pressure and heart rate in either direction. On paper, mixing those with caffeine could either bump up jittery effects (a racing heart, anxiety, higher blood pressure, trouble sleeping) or work against them. Honestly, the direction isn't predictable.

I want to be straight with you: this is a theoretical concern based on weak, anecdotal evidence, not solid studies on fever bark itself. Still, fever bark is a potent herb I'd be cautious with regardless. Please run it by your pharmacist or doctor before combining the two.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.

Here's the thing to know about this combo. Fiorinal with Codeine already contains aspirin, which thins your blood and makes platelets less sticky. Fever bark contains a yohimbine-like compound that, in lab studies, also seemed to make platelets less likely to clump together. So in theory, stacking the two could nudge your bleeding risk a little higher. That might show up as easier bruising, a nosebleed that won't quit, or bleeding in the stomach (aspirin can already irritate the gut).

I want to be honest though: this is rated minor and is based only on test-tube and animal data, not real people taking them together. It's a theoretical concern, not something I'd panic over. Still, fever bark's alkaloids can affect blood pressure and heart rate too, so I'd run it past your pharmacist before mixing them.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
