Effient falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the twist worth knowing: Effient (prasugrel) is a prodrug, meaning it's inactive until your body switches it on. The interesting part is that prasugrel mostly relies on other enzymes (like CYP3A4 and CYP2B6) to become active, not CYP2C19 in any major way. So even though goji may gently slow CYP2C19 in lab studies, this isn't a pathway prasugrel depends on heavily.
This concern is theoretical and comes only from test-tube research, with nothing reported in actual people. Goji as a food is very unlikely to matter. Concentrated goji supplements are less studied, so since Effient affects your blood's ability to clot, it's smart to mention any goji products to your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the honest picture with goji and Effient (prasugrel). The concern on file is that goji may slow down a liver enzyme called CYP2C9. But this matters far less for Effient than it does for a drug like warfarin. Effient is a prodrug, meaning your body has to activate it, and that activation relies mainly on CYP3A4 and CYP2B6, not really CYP2C9. So the listed mechanism doesn't line up neatly with how Effient actually works.
On top of that, this interaction is theoretical and based only on lab tests and anecdotes, so the real-world risk looks low. Eating goji berries as food is very unlikely to cause trouble. Still, since Effient already raises bleeding risk, run any concentrated goji juice or supplement past your pharmacist or doctor first.

In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the thing about Effient (prasugrel): it's a prodrug, meaning it has to be switched on by your body's enzymes before it can do its job of keeping platelets from clumping. CYP3A4 is one of the enzymes involved in turning it into its active form. So if goji slowed that enzyme down, the worry would actually be the opposite of build-up: you could get a little less of the active drug, which in theory might make it work less well at preventing clots.
That said, I don't want you to lose sleep over this. The whole concern comes from lab (test tube) studies on goji juice, and it has never been reported in real people. Eating goji berries as a food is very unlikely to matter. Before starting a concentrated goji supplement, just run it by your pharmacist or doctor.

What Is Prasugrel Used For?
Brand name: Prasugrel
Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.
Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
