Fedratinib Hydrochloride falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.

Here's the gist for fedratinib (Inrebic): your liver uses an enzyme called CYP2C19 to help clear this drug from your body. In lab studies, goji berry extracts seem to slow that enzyme down. If that same effect happened in people, your fedratinib levels could creep up, which might mean more of its usual side effects like nausea, diarrhea, low blood counts, or feeling unusually tired.

The important thing to know is that this is theoretical, seen only in test-tube research and never actually reported in humans. Eating goji berries as food is very unlikely to be a problem. Concentrated goji supplements, juices, or tinctures are the bigger question mark. Since fedratinib is a serious cancer medicine, please check with your oncologist or pharmacist before adding any goji supplement.

Your body clears fedratinib mainly through a liver enzyme called CYP3A4. Lab studies suggest goji berry juice can slow that enzyme down a bit. If that happened in real life, fedratinib could clear more slowly and build up in your blood, which may mean stronger effects and a higher chance of side effects like nausea, diarrhea, vomiting, fatigue, or muscle aches.

Here is the reassuring part: this comes only from test-tube research and has not been seen in actual patients, so it is theoretical. Still, fedratinib is a serious cancer drug with a real safety profile, so I would not start a concentrated goji supplement without checking first. Eating goji berries as a food is much less likely to matter. Please run any goji supplement by your oncologist or pharmacist.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
