CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

pioglitazone, metformin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with ActoPlus Met (which combines pioglitazone and metformin). Lab studies suggest goji berry might block an enzyme called CYP3A4, and pioglitazone is partly cleared by that enzyme. In theory, slowing it down could let pioglitazone build up a bit, which might nudge your blood sugar lower or add to side effects like fluid retention, swelling in your legs, or weight gain. The metformin half isn't really handled by this enzyme, so it's less of a concern here.
The big caveat: this comes only from test-tube research and has never actually been reported in people, so I wouldn't lose sleep over a little goji in your food. If you're thinking about concentrated goji supplements, just run it by your pharmacist or doctor first.

ANTIDIABETES DRUGS
2 of 2pioglitazone, metformin also falls under ANTIDIABETES DRUGS.
Animal and in vitro research show that goji root bark and fruit polysaccharides might have hypoglycemic effects. However, clinical research has only shown that taking goji fruit polysaccharides with or without antidiabetes drugs modestly reduces postprandial glucose when compared with control, with no reports of hypoglycemia.

Here's the simple version: goji berries, especially concentrated polysaccharide supplements, may nudge your blood sugar down a little on their own. Since ActoPlus Met (the combo of pioglitazone and metformin) is already working to lower your glucose, stacking the two could add up to a slightly bigger drop. The good news is this is rated a minor interaction, and human studies only showed a modest dip in after-meal sugar with no reports of true low blood sugar.
Eating goji berries as a food is very unlikely to cause trouble. With concentrated capsules or extracts, just keep an eye out for signs of low sugar like shakiness, sweating, or dizziness, and check your readings if you start them. It's worth a quick chat with your pharmacist or doctor before adding a goji supplement.
