Tapentadol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: goji berries might block a liver enzyme called CYP2C19, and in theory that could slow down how your body clears tapentadol. If tapentadol hangs around longer, you might feel more of its effects, like extra drowsiness, dizziness, constipation, nausea, or slowed breathing. Tapentadol works as-is (it isn't a prodrug that needs activating), so more of it in your system means stronger opioid effects, not weaker ones.
That said, I want to be honest about the evidence. This comes only from lab and test-tube studies, and it has never actually been reported in people. Eating goji berries as a normal food is unlikely to cause trouble. Concentrated goji supplements, tinctures, or juices are the bigger question mark. If you use those, just check with your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for goji and your tapentadol. The concern on file is that goji may slow down a liver enzyme called CYP2C9. In theory, blocking that enzyme could let a drug it clears build up a bit higher in your body. With tapentadol, higher levels could mean stronger opioid effects, like more drowsiness, dizziness, nausea, constipation, or slowed breathing.
Now the reassuring part: this is mostly lab and case-report evidence, and tapentadol actually leans more on a different pathway (glucuronidation) than on CYP2C9, so the real-world effect is likely small. Everyday goji berries as a food are very unlikely to matter. Concentrated goji teas, juices, or supplements are where I'd be more cautious. Please run it by your pharmacist or doctor before combining them.

What Is Tapentadol Used For?
Brand names: Nucynta, Nucynta ER
Tapentadol tablets (including Nucynta and Tapentadol Hydrochloride tablets) are used for severe acute pain in adults when other treatments are not adequate. Nucynta tablets are also approved for acute pain in children aged 6 and older who weigh at least 40 kg.
Nucynta ER and tapentadol extended-release tablets are used for severe, ongoing pain that requires around-the-clock opioid treatment and cannot be managed with other options, including severe neuropathic pain (nerve pain) caused by diabetic peripheral neuropathy in adults. Extended-release formulations are not intended for as-needed use.
