Green Tea and Aspirin, Butalbital, Caffeine, Codeine Phosphate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Green Tea interacts with whole categories of medication. Aspirin, Butalbital, Caffeine, Codeine Phosphate falls into 3 of those categories, so it can interact with Green Tea in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 3

Why this applies to Aspirin, Butalbital, Caffeine, Codeine Phosphate

Aspirin, Butalbital, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Green Tea interacts with.

Theoretically, green tea may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Conflicting reports exist regarding the effect of green tea on bleeding risk when used with anticoagulant or antiplatelet drugs; however, most evidence suggests that drinking green tea in moderate amounts is unlikely to cause a significant interaction. Green tea contains small amounts of vitamin K, approximately 7 mcg per cup. Some case reports have associated the antagonism of warfarin with the vitamin K content of green tea. However, these reports are rare, and very large doses of green tea (about 8-16 cups daily) appear to be needed to cause these effects. Furthermore, the catechins and caffeine in green tea are reported to have antiplatelet activity.

The piece of Fiorinal with Codeine that matters here is the aspirin, which already thins the blood a bit by making platelets less sticky. Green tea contains plant compounds (catechins) and caffeine that may have a mild blood-thinning effect of their own, so in theory the two together could nudge your bleeding risk up. That might show up as easier bruising, a nosebleed, or a cut that takes longer to stop.

In real life this is unlikely to be a problem, and the evidence is weak (mostly lab and animal data). A cup or two of green tea is no big deal for most folks. Concentrated green tea extract pills are where I'd be a little more careful. If you bruise easily or notice unusual bleeding, check in with your pharmacist or doctor.

STIMULANT DRUGS

2 of 3

Aspirin, Butalbital, Caffeine, Codeine Phosphate also falls under STIMULANT DRUGS.

Theoretically, concomitant use might increase stimulant adverse effects.
Green tea contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about Fiorinal with Codeine: it already contains caffeine right in the tablet. Green tea brings its own caffeine to the table too. So when we talk about an interaction here, it's really about stacking caffeine on top of caffeine. The more you pile on, the more likely you are to feel that jittery, wired feeling, a racing or pounding heart, trouble sleeping, or an upset stomach.

This is rated as a theoretical concern based on expert opinion, not strong studies, so a cup or two of green tea probably won't cause a problem for most folks. Concentrated green tea extract pills are a different matter, since they can pack in much more caffeine. If you notice you're feeling extra jittery or your sleep is off, chat with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Aspirin, Butalbital, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Good news here: drinking green tea alongside Fiorinal with Codeine isn't something you need to lose sleep over. The concern on file is that green tea might nudge the CYP3A4 enzyme system, which helps handle some of the ingredients in this combination pill. But this is based only on lab and animal data, and in real people green tea hasn't been shown to meaningfully change how these drugs work.

So in everyday cups of tea, don't expect green tea to change your pain relief or make side effects like drowsiness, constipation, or stomach upset any worse. Concentrated green tea extract supplements are a bigger unknown, so if you take those, mention it to your pharmacist or doctor, especially since this medicine already affects the liver and stomach.

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
