Gymnema and Abrocitinib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Abrocitinib

Abrocitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses an enzyme called CYP2C9 to help process abrocitinib, and gymnema may tinker with that same enzyme. The catch is the direction isn't clear. Lab and animal studies pull both ways, with gymnema sometimes speeding the enzyme up and sometimes slowing it down. So in theory, gymnema could either lower your abrocitinib levels (less control of your eczema, more itching and flares) or raise them (a higher chance of side effects like headache, nausea, acne, or a dip in your blood counts).

Honestly, this is a theoretical concern based only on test-tube and rat data, not real patients, so I wouldn't lose sleep over it. Still, since abrocitinib affects your immune system, it's worth a quick word with your pharmacist or doctor before adding gymnema, especially in concentrated supplement doses.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Abrocitinib page

“Bleeding is where I'd focus first with abrocitinib (Cibinqo), an eczema pill that can itself lower platelet counts. Of the 290 supplements and herbs with an interaction on file, 202 have blood-thinning or antiplatelet activity, and two of the three major-rated entries, danshen and piracetam, sit in that group.”

About Gymnema

What is Gymnema?

Gymnema
  • Blood sugar support
  • Type 2 diabetes
  • Sugar cravings
  • Weight management
  • Cholesterol support

Gymnema is an Ayurvedic herb best known for possibly helping lower blood sugar and reducing the taste of sweetness on the tongue. Some early human studies are encouraging for blood sugar support, but the evidence is still limited and not strong enough to replace standard treatment. If you have diabetes or take other medicines, talk with your pharmacist or doctor before using it.

About Abrocitinib

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
