Why Are There 2 Interactions?
Gymnema interacts with whole categories of medication. Alogliptin falls into 2 of those categories, so it can interact with Gymnema in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
ANTIDIABETES DRUGS

Alogliptin falls under ANTIDIABETES DRUGS — the group Gymnema interacts with.
Gymnema reduces blood glucose levels in some human and animal research. In human studies, it has been shown to enhance the blood glucose lowering effects of hypoglycemic drugs. However, other research in adults with prediabetes or metabolic syndrome suggests that gymnema does not reduce fasting levels of blood glucose. Until more is known, monitor blood glucose levels closely.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both Gymnema and your alogliptin (Nesina) work to bring blood sugar down. Gymnema may have its own mild blood sugar lowering effect, so stacking the two together could push your numbers lower than alogliptin alone would. That raises the chance of hypoglycemia, which can show up as shakiness, sweating, dizziness, a fast heartbeat, or feeling confused or unusually hungry.
To be fair, the evidence here is mixed and mostly theoretical. Some studies show Gymnema adds to the effect, while others in people with prediabetes saw no real change. Alogliptin by itself rarely causes lows, but adding Gymnema tips the odds. If you'd like to try it, check your blood sugar more closely and chat with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Alogliptin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the good news on this pairing: the worry about Gymnema slowing the CYP3A4 enzyme and letting alogliptin (Nesina) build up is mostly theoretical. It comes from a single test-tube study using rat liver tissue. When researchers looked at human liver tissue and live animals, Gymnema didn't really change CYP3A4 activity at all. On top of that, alogliptin barely relies on this pathway to begin with, since most of it leaves the body unchanged through your kidneys.
The thing actually worth keeping an eye on is your blood sugar. Both alogliptin and Gymnema can lower it, so together they could nudge your numbers lower than expected. That's usually mild with a DPP-4 drug like this one, but if you add Gymnema, check your sugars and let your pharmacist or doctor know.
