Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's the short version: your body uses a liver enzyme called CYP2C9 to help clear bosentan from your system. Gymnema may tug on that enzyme, but the picture is muddy. In animal studies it seemed to speed the enzyme up (which could lower bosentan levels and weaken its effect on your lung pressure), while lab dish studies suggest it could slow the enzyme down (which could raise levels and bring on more side effects like headache, flushing, swelling in the legs, or liver irritation).

The honest truth is this is theoretical, based only on animal and test-tube work, not real people. Bosentan is a drug we watch closely, so before adding gymnema, please run it by your pharmacist or doctor first.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the short version: your body clears Bosentan using a liver enzyme called CYP3A4, and there's a theory that Gymnema could slow that enzyme down a little. If that happened, Bosentan levels might creep up, which could mean more of its usual side effects like headache, flushing, swelling in the legs, or low blood pressure. Bosentan is not a prodrug, so this is about the drug building up, not weakening.

The good news is the evidence here is very thin. The only hint of blocking came from a rat liver study, while human and animal studies showed no real effect on this enzyme. So in practice this is unlikely to cause a problem. Still, since both can affect blood sugar and Bosentan needs regular liver monitoring, please check with your pharmacist or doctor before adding Gymnema.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
