Why this applies to Entrectinib

Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the quick version: your body clears entrectinib using a liver enzyme called CYP3A4. Gymnema has been looked at to see if it slows that same enzyme down. On paper, if gymnema blocked CYP3A4, entrectinib could build up a bit and you might see more of its usual side effects, things like dizziness, fatigue, swelling, changes in heart rhythm, or numbness and tingling.

But honestly, this concern is very weak. The only hint of a problem came from one test-tube study using rat liver. Studies done with human liver tissue and live animals showed gymnema did not meaningfully change CYP3A4. So a real-world interaction is unlikely. Still, because entrectinib is a serious cancer medicine, please run any supplement past your oncology team or pharmacist before starting it.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
