Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP1A2.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.

Here's the gist for Fycompa (perampanel). Your body uses several liver enzymes to clear this seizure medicine, and one of them is CYP1A2. Lab and animal studies hint that gymnema may slow down that same enzyme. In theory, that could let perampanel build up a bit more than usual, which might mean stronger effects like extra drowsiness, dizziness, trouble with balance, or mood changes (irritability or anger have been reported with this drug).

I want to keep this in perspective: this is theoretical, based only on test-tube and animal work, not real human data with perampanel. It may turn out to matter very little. Still, since perampanel can already make you sleepy, it's worth a quick chat with your pharmacist or doctor before adding gymnema.

Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's the short version: your seizure medicine, perampanel (Fycompa), is partly cleared by a liver enzyme called CYP2C9, and gymnema may nudge that enzyme. The catch is that the evidence here is only from lab dishes and animal studies, not from people, so this is mostly theoretical. In animals, gymnema mostly seemed to speed up this enzyme, which could in theory lower perampanel levels and make it less effective. That matters because weaker seizure control can mean breakthrough seizures.

To muddy things further, other lab work suggests gymnema might block the same enzyme, which could push perampanel higher and bring more dizziness, sleepiness, or unsteadiness. Because we can't predict the direction with confidence, please check with your doctor or pharmacist before adding gymnema rather than changing anything on your own.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist: your body clears perampanel (Fycompa) using a liver enzyme called CYP3A4. The thought is that gymnema might slow that enzyme down a little. If it did, perampanel could build up slightly and you might notice more of its usual effects, like drowsiness, dizziness, trouble with balance, or irritability and mood changes.

I want to be honest about how shaky this concern is. It comes only from a test-tube study using rat liver, while studies in human liver tissue and live animals showed gymnema did not change CYP3A4 activity. So a real-world problem here is unlikely. Still, perampanel is a seizure medicine, so before adding gymnema, run it by your pharmacist or doctor first.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
