Why this applies to Progestin, Oral Contraceptive

Progestin, Oral Contraceptive falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the short version: your drospirenone pill (Slynd) is partly cleared by a liver enzyme called CYP3A4. The thinking is that gymnema might slow that enzyme down a bit, which could let a little more of the progestin hang around. In theory that might mean a touch more spotting, breast tenderness, or nausea, but honestly this is mostly hypothetical.

The evidence here is very weak. It comes from a test-tube study in rat tissue, and other lab and animal work showed no effect at all on this enzyme. So I wouldn't lose sleep over it. The bigger thing to know is that gymnema lowers blood sugar, which matters more if you have diabetes. Just give your pharmacist a heads-up before starting it.

  • Progestin-only (drospirenone) oral contraceptives are used to prevent pregnancy. Progestin is a female hormone. It works by preventing the release of eggs from the ovaries (ovulation) and changing the cervical mucus and the lining of the uterus. Progestin-only oral contraceptives are a very effective method of birth control, but they do not prevent the spread of of human immunodeficiency virus (HIV, the virus that causes acquired immunodeficiency syndrome [AIDS]) and other sexually transmitted diseases.
