Why this applies to Ruxolitinib

Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Your body relies on a liver enzyme called CYP3A4 to clear ruxolitinib (Jakafi, Opzelura). The concern here is that gymnema might slow that enzyme down a little. If it did, ruxolitinib could build up slightly, which in theory could mean more of its known side effects, like low blood counts (anemia, low platelets), bruising, dizziness, or a higher chance of infections.

The good news: this worry is very weak. It comes from one test-tube study using rat liver tissue, while studies in human liver tissue and live animals showed no real effect. So a meaningful interaction is unlikely. Since ruxolitinib treats serious conditions and gymnema may also lower blood sugar, please run any supplement by your pharmacist or doctor first.

About Ruxolitinib

What Is Ruxolitinib Used For?

Brand names: Jakafi, Opzelura

Ruxolitinib is approved to treat several serious blood and immune conditions in adults, and some in patients as young as 12. It is used for myelofibrosis (a bone marrow disorder that disrupts normal blood cell production), polycythemia vera (a condition where the bone marrow makes too many red blood cells), and two forms of graft-versus-host disease (GVHD) — a complication that can occur after a stem cell transplant.

For polycythemia vera, it is specifically for patients who have not responded well to, or cannot tolerate, a medication called hydroxyurea. For chronic GVHD, it is used after at least one or two other treatments have failed.
