Sildenafil Citrate falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase or decrease levels of drugs metabolized by CYP2C9.
Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.

Here's the honest picture with Gymnema and sildenafil (Viagra). Your body partly relies on a liver enzyme called CYP2C9 to clear sildenafil. Gymnema may nudge that enzyme in either direction, so in theory it could either speed up or slow down how fast sildenafil is broken down. If it speeds clearance, the drug might work less well. If it slows clearance, levels could rise, raising the chance of side effects like headache, facial flushing, a stuffy nose, dizziness, or a bigger drop in blood pressure.

The reassuring part: this is only based on lab and animal studies, not real people, so it may not amount to much. Still, since gymnema can also lower blood sugar, it's worth a quick chat with your pharmacist or doctor before combining them.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist for Viagra. Your body clears sildenafil mainly through a liver enzyme called CYP3A4. The thinking is that gymnema might slow that enzyme down a little, which in theory could let sildenafil hang around longer and feel a bit stronger. That could mean more of its usual side effects, like headache, facial flushing, a stuffy nose, or lightheadedness.

That said, I wouldn't lose sleep over this one. The only hint of an effect comes from a single test-tube study using rat liver, while other lab and animal research showed no change at all. So this is theoretical and unlikely to matter much in real life. Still, since gymnema can also lower blood sugar, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Sildenafil Used For?

Brand name: Viagra

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.

Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
