Why this applies to Ondansetron

Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hops interacts with.

Hops extract does not seem to affect the metabolism of CYP1A2 substrates.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

Here's the good news on combining hops with ondansetron (Zofran): this pairing looks like a non-issue for most people. The concern on paper is that flavonoids in hops might slow down CYP1A2, one of the liver enzymes that helps clear ondansetron. In theory, blocking that enzyme could let a little more drug linger. But ondansetron isn't a prodrug, and it leans on several enzymes (mainly CYP3A4), not just CYP1A2, so any small shift here isn't likely to matter.

Importantly, this worry comes only from lab (test tube) data. In actual people, a standardized hops extract did not change levels of a CYP1A2 marker. So I wouldn't expect more drowsiness, headache, or constipation from this combo. Still, if you take both regularly, mention it to your pharmacist.

Ondansetron also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the short version: ondansetron (Zofran) is partly broken down by a liver enzyme called CYP3A4, and hops might nudge that enzyme to work a little faster. In theory, speeding up the breakdown could lower ondansetron levels slightly, which in turn could make it a touch less effective at keeping nausea and vomiting away.

But I want to keep this in perspective. This concern comes mostly from lab and animal data, and a human study found only about an 8% change in a similar drug, which isn't enough to matter in real life. Ondansetron is not a prodrug, so it doesn't get flipped around the way some medicines do. For most people this pairing is fine, but if you're relying on Zofran for serious nausea, just run it by your pharmacist or doctor first.

Hops are most often used for sleep and mild anxiety, frequently combined with valerian, but the human evidence is limited and not very strong. They are generally well tolerated when used short-term, though they may cause drowsiness, and should be avoided during pregnancy and breastfeeding because their safety is unknown.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
