Why this applies to Lesinurad

Lesinurad falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for lesinurad (Zurampic): your liver uses an enzyme called CYP2C9 to help process this gout medicine. Limonene, the oil from citrus peels, may gently speed that enzyme up (a process called induction). If that happened, your body could clear lesinurad a little faster, which in theory might make it slightly less effective at lowering your uric acid. The practical worry would be less control over gout, meaning a higher chance of flare-ups with joint redness, swelling, and pain.

That said, this is based only on lab and animal data, not real human reports, so it may not amount to much. Small amounts of citrus in food are very unlikely to matter. Before starting a concentrated limonene supplement, just check with your pharmacist or doctor.

What Is Lesinurad Used For?

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
