Why this applies to Perampanel

Perampanel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C9 (CYP2C9), as well causing its induction. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking limonene. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for Fycompa (perampanel). Your seizure medicine is partly cleared by a liver enzyme called CYP2C9. Lab work suggests concentrated limonene supplements might speed that enzyme up (induce it). If that happened in real life, your body could clear perampanel a little faster, which in theory could lower its levels and make it less effective at controlling seizures.

I want to be honest about how shaky this is. This comes only from test tube and animal data, and it has never actually been reported in people. The limonene you get from eating an orange or a splash of lemon is very unlikely to matter. With perampanel, even small changes in seizure control are worth taking seriously, so before adding a concentrated limonene supplement, please check with your pharmacist or neurologist.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
