This combination product pairs aspirin and caffeine with codeine, and it's the codeine piece that matters most here. Codeine is an opioid that can make you drowsy on its own. Melatonin also nudges your body toward sleep. Put them together and the sleepy effect can stack up, so you might feel more groggy, slow, or lightheaded than you'd expect.
This is a theoretical concern based on how each one works, not something proven in studies, so don't panic. Still, be careful with driving or anything that needs you sharp until you know how you react. The caffeine in this product won't reliably cancel the sedation. Here's the gist for this combo pill, which mixes aspirin, caffeine, and codeine. The interaction we have on file isn't about how melatonin and codeine are broken down. It's a seizure threshold concern. In theory, melatonin combined with medicines that can lower the brain's resistance to seizures might slightly raise the chance of seizure activity. The codeine portion is the part of this product that can nudge the seizure threshold, especially at higher doses.
To be honest, this is a theoretical caution based on pharmacology, not strong proof. The signal is mostly seen in certain children with neurological conditions. For most adults taking this short-term, the risk is low. Still, if you have a seizure history, check with your pharmacist or doctor before pairing them.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
In this combo pill, the piece that overlaps with melatonin is the caffeine, which your liver clears using an enzyme called CYP1A2. On paper, melatonin may slow that enzyme a little, so caffeine could hang around a bit longer. If anything, you might feel slightly more of caffeine's usual effects, like jitteriness, a racing heart, or trouble falling asleep, which is a little ironic since you may be taking melatonin to sleep.
The good news: this is a theoretical concern based on how these substances behave, not strong real-world proof, and it isn't dangerous for most people. The aspirin and codeine parts aren't the issue here. Still, it's worth a quick chat with your pharmacist or doctor before pairing them.

ANTICOAGULANT/ANTIPLATELET DRUGS

There are isolated case reports of minor bleeding and decreased prothrombin activity in people taking melatonin with warfarin (Coumadin). The mechanism, if any, of this interaction is unknown. Taking melatonin orally seems to decrease coagulation activity within one hour of dosing in healthy men.

Here's the thing to keep an eye on with this combination product: the aspirin portion already thins the blood a bit by making your platelets less sticky. There's a theory that melatonin may also nudge your blood toward clotting a little more slowly. Stack the two together and, at least on paper, you could see a slightly higher chance of bruising or bleeding, like nosebleeds, bleeding gums, or a cut that takes longer to stop.
I want to be honest that this is mostly theoretical, built on a few isolated case reports (mostly with warfarin), not strong proof. For most people taking occasional doses, it's likely minor. Still, if you bruise easily, notice unusual bleeding, or take this regularly, run it by your pharmacist or doctor first.

CAFFEINE

Some evidence suggests that caffeine consumption can decrease endogenous melatonin levels, while other evidence suggests that caffeine increases endogenous melatonin levels. When administered in combination with melatonin supplements, caffeine seems to increase melatonin effects and levels. The reason for this discrepancy is not completely clear. Part of the discrepancy may result from the fact that caffeine can inhibit melatonin synthesis as well as inhibit melatonin metabolism. By functioning as an adenosine receptor antagonist, caffeine may indirectly inhibit the synthesis of melatonin. Conversely, because melatonin and caffeine are both metabolized by cytochrome P450 1A2 (CYP1A2) enzyme, concomitant use of melatonin and caffeine may reduce the metabolism of melatonin, resulting in higher serum levels.

Here's the interesting wrinkle with this combo: the part that talks to melatonin is the caffeine in your Anacin with Codeine, not the aspirin or codeine. Both caffeine and melatonin get processed by the same liver enzyme (CYP1A2). When they ride together, the caffeine can slow melatonin's breakdown, so your melatonin levels may run a bit higher than expected. The practical result is usually just more drowsiness or grogginess than you planned on.
This is rated moderate and the evidence is on the weaker side, so don't panic. Just keep in mind the caffeine in this product can also make it harder to fall asleep, which works against the melatonin. If you take both regularly, run it by your pharmacist or doctor first.

Here's the wrinkle with this combo: the codeine part of your Anacin w/ Codeine is a prodrug. Your body has to convert codeine into morphine using a liver enzyme called CYP2D6 before you get much pain relief. Lab studies hint that some batches of melatonin may slow that same enzyme down. If that happened, less codeine would turn into morphine, so you might notice weaker pain relief rather than more side effects.
I want to be honest about the strength of this, though. This is only a theoretical concern from test-tube research, the rating is minor, and the aspirin and caffeine pieces aren't really affected. For most people taking melatonin short-term for sleep, this isn't a big deal. Still, if your pain control changes, mention it to your pharmacist or doctor.
