Why this applies to Nirmatrelvir, Ritonavir
Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's something worth knowing if you're taking Paxlovid and reaching for melatonin to sleep. Some lab studies suggest melatonin might mildly slow down the CYP3A4 enzyme, which is one of the pathways your body uses to clear certain drugs. In theory, that could nudge drug levels up a bit. But honestly, the bigger player here is the ritonavir inside Paxlovid itself, which is a powerful CYP3A4 blocker by design.
So if anything, it's the Paxlovid affecting the melatonin, not the other way around. You may find the melatonin feels a little stronger, meaning more grogginess or daytime drowsiness. This is theory based on lab data only, so I wouldn't lose sleep over it. Still, mention it to your pharmacist, especially since Paxlovid interacts with many medications.
