ANTIHYPERTENSIVE DRUGS

Bisoprolol, Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Melatonin interacts with.
Some clinical research suggests that taking melatonin decreases blood pressure in healthy adults. Also, melatonin seems to lower systolic and diastolic blood pressure in individuals with high blood pressure at nighttime or untreated essential hypertension. However, melatonin seems to worsen blood pressure in patients who are taking antihypertensive medications. Immediate-release melatonin 5 mg at night in combination with nifedipine GITS (Procardia XL) increases systolic blood pressure an average of 6.5 mmHg, diastolic blood pressure by an average of 4.9 mmHg, and heart rate by 3.9 bpm. Also, results from animal research suggest that melatonin reduces the effectiveness of certain antihypertensive drugs, including methoxamine and clonidine.

Ziac combines bisoprolol (a beta blocker) with hydrochlorothiazide (a water pill), and together they are working to keep your blood pressure under control. Here is the wrinkle with melatonin: even though melatonin on its own can nudge blood pressure down, some research suggests it can actually push back against blood pressure medicines and blunt how well they work. In one study with another blood pressure drug, people taking melatonin saw their numbers tick up a bit, along with a small bump in heart rate.
So the realistic concern here is slightly less blood pressure control, not a dangerous drop. This is rated moderate and is still somewhat theoretical for your exact combo. If you want to try melatonin for sleep, that is usually reasonable, just check in with your pharmacist or doctor first and keep an eye on your home blood pressure readings.

SEIZURE THRESHOLD LOWERING DRUGS

Some clinical evidence suggests that melatonin may increase the frequency of seizures in certain patients, particularly children with neurological disabilities.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest version: this flag is pretty theoretical when it comes to your Ziac (bisoprolol plus hydrochlorothiazide). The concern is that melatonin might slightly lower the seizure threshold in some people, and so do a long list of medicines lumped into that category. But bisoprolol (a beta blocker for blood pressure and heart rate) and hydrochlorothiazide (a water pill) are not classic seizure-triggering drugs, and this is graded as theoretical, not something seen often in real life.
For most adults taking Ziac for blood pressure, adding melatonin for sleep is unlikely to cause a problem. If you have a seizure history or a neurological condition, just run it by your pharmacist or doctor first to be safe.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
3 of 3Bisoprolol, Hydrochlorothiazide also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for Ziac, which combines bisoprolol (a beta-blocker) with hydrochlorothiazide (a water pill). Lab studies hint that some batches of melatonin can slow down a liver enzyme called CYP2D6. Bisoprolol is partly handled by that enzyme, so in theory melatonin could let a little more of it linger in your system. If that happened, you might notice the beta-blocker effects feeling a touch stronger, things like a slower pulse, more tiredness, or feeling lightheaded when you stand up.
I want to keep this in perspective, though. This is only a theoretical concern from test-tube research, the rating is minor, and bisoprolol is not heavily dependent on CYP2D6. For most people short-term melatonin is fine here, but if you notice unusual fatigue or dizziness, check in with your pharmacist or doctor.
