Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.
Animal research shows that indole-3-carbinol induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill carries three active ingredients, and the part that interacts here is the caffeine. Your liver clears caffeine using an enzyme called CYP1A2, and indole-3-carbinol may speed that enzyme up. In theory that means caffeine could be flushed out a little faster, so you might notice less of its usual lift or be more likely to feel groggy as it wears off. The aspirin and codeine portions are not really affected by this particular pathway.
Keep in mind this is based only on lab and animal studies, not solid human data, so the real-world effect is likely small. The amount you get from eating broccoli or cabbage almost certainly won't matter. If you take a concentrated I3C supplement regularly, just mention it to your pharmacist or doctor.

In vitro research shows that indole-3-carbinol inhibits platelet aggregation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the part that matters for this combo product: it contains aspirin, which is itself a blood thinner that keeps platelets from clumping together. The concern on file is that indole-3-carbinol may do something similar. In lab dish studies (not yet in people), it appeared to slow platelet clumping too. Stack that on top of aspirin and you could, in theory, nudge up your bleeding risk.
In real life this might look like easier bruising, nosebleeds, or bleeding that takes longer to stop. To be fair, this is theoretical and based only on test-tube work, so it's far from proven. Everyday broccoli or cabbage is not the worry. Concentrated I3C supplements are. Please run it by your pharmacist or doctor first.
