Ivacaftor, Lumacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the short version: the ivacaftor part of Orkambi is partly cleared by a liver enzyme called CYP3A4. In some lab studies, certain batches of melatonin slowed that same enzyme down a bit. In theory, that could let ivacaftor levels creep up slightly, which might mean a touch more in the way of side effects like headache, stomach upset, or feeling unusually tired.

I want to be honest about how weak this is, though. It comes only from test-tube research, not from real people, and the concern is rated minor. Lumacaftor actually tends to speed CYP3A4 up, which muddies things further. For most folks this won't cause trouble, but since cystic fibrosis therapy matters so much, just give your pharmacist or CF team a heads-up before adding melatonin.

What Is Lumacaftor and Ivacaftor Used For?

Brand name: Orkambi

Orkambi is approved to treat cystic fibrosis (CF) in patients age 1 year and older who have two copies of the F508del mutation in the CFTR gene — one on each chromosome. This is called being homozygous for F508del. If a patient's genetic type is unknown, a lab test approved by the FDA should confirm the mutation before starting treatment.

Orkambi has not been proven safe or effective for people with CF who have a different mutation pattern. It is only for the F508del/F508del genotype.
