Jequirity and Aspirin, Caffeine, Codeine Phosphate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Jequirity interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 2 of those categories, so it can interact with Jequirity in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

1 of 2

Why this applies to Aspirin, Caffeine, Codeine Phosphate

Aspirin, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Jequirity interacts with.

Evidence from in vitro research suggests that constituents of jequirity beans can inhibit platelet aggregation. Theoretically, jequirity might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs. Some anticoagulant or antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to know about this combination: your medicine already contains aspirin, which thins the blood by keeping platelets from clumping together. In lab studies, compounds from jequirity beans appear to slow platelet activity too. So in theory, pairing them could add up and raise your risk of bleeding or easy bruising, things like nosebleeds, bleeding gums, or a cut that takes longer to stop.

I want to be honest, though: this concern is theoretical and based only on test-tube research, not real patient cases. The bigger worry with jequirity is its seeds, which contain abrin, a genuinely dangerous poison. My honest advice is to steer clear of jequirity entirely, and check with your pharmacist or doctor before adding any new supplement to an aspirin-containing product.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Aspirin, Caffeine, Codeine Phosphate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that jequirity extract inhibits CYP3A4 enzymes. Theoretically, taking jequirity may inhibit the metabolism of CYP3A4 substrates.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: jequirity seeds are genuinely poisonous (they contain abrin, a toxin much like ricin), so this isn't something you should be taking at all. Put the seed concern aside, and the interaction itself is very weak. It comes only from lab studies suggesting jequirity might slow down a liver enzyme called CYP3A4, which helps process this combo medicine.

One thing worth noting: the codeine part is a prodrug. Your body has to convert it (mainly through a different enzyme, CYP2D6) into morphine for pain relief, so the CYP3A4 angle plays only a small role here. In theory, slowing CYP3A4 could let a bit more drug linger, which might mean more drowsiness, constipation, or stomach upset. But this is theoretical and rated as unlikely. Please run anything you're taking past your pharmacist or doctor.

From our pharmacist’s overview on the Aspirin, Caffeine, Codeine Phosphate page

“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”

  • Traditional infectious-disease remedies

Jequirity (Abrus precatorius) is a tropical plant whose bright red, black-tipped seeds contain abrin, one of the most toxic plant proteins known. Despite a long history in traditional medicine, the seeds are dangerously poisonous and there is no reliable evidence that it is safe or effective. It should not be used as a supplement or medicine.
