Jequirity and Rifampin

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Rifampin

Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Jequirity interacts with.

In vitro research shows that jequirity extract inhibits CYP3A4 enzymes. Theoretically, taking jequirity may inhibit the metabolism of CYP3A4 substrates.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: I would steer you away from jequirity entirely. Its seeds contain abrin, a toxin in the same family as ricin, and there is no safe or proven reason to take it. That alone is the headline here, far more than any drug interaction.

As for the lab finding, test-tube studies suggest jequirity may block CYP3A4, an enzyme that helps process many medicines. In theory that could slow the breakdown of a CYP3A4 substrate and let it build up. With rifampin, though, this is very weak, theoretical evidence only (no human data), so I would not lose sleep over it. The real issue is the plant's toxicity. Please check with your pharmacist or doctor before adding anything new to your TB treatment.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

Jequirity (Abrus precatorius) is a tropical plant whose bright red, black-tipped seeds contain abrin, one of the most toxic plant proteins known. About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.

It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.

Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
